The list of physicians generated by your search criteria is based on the ZIP code or city that you entered. You will have the option to sort search results by proximity to the ZIP code or city that you entered or by listing UroLift™ Center of Excellence (COE) physicians at the top. UroLift™ COE physicians have fulfilled the designation requirements for the UroLift™ COE Program.

The information and content in the Physician Locator are provided solely by the physicians. This list was created to support patients who want to learn more about the UroLift™ System by helping connect them to physicians who currently have demonstrated qualifications and interest in providing quality care with the UroLift™ System. All physicians have been given the choice to enable a link to their email address within the listing associated with their name.

Physicians listed on the Physician Locator, with or without COE Program designation, pay no fee for inclusion. Some physicians on this list may purchase products from, provide consulting services to and/or be a party to a co-marketing agreement with Teleflex. Teleflex makes no representations or warranties regarding, and shall not be responsible for, the competencies or skill level of any of the physicians listed on the Physician Locator or designated as COEs or the quality of their procedural outcomes. You and your physician must determine if the UroLift™ System treatment is the right procedure for you.

Teleflex reserves the right to remove and/or exclude any physician from the Physician Locator, with or without COE Program designation, immediately and without notice in its sole discretion in the event that Teleflex becomes aware of (i) any alleged or suspected fraud or abuse or violation of law, arising out of any activity involving the physician, (ii) any alleged or suspected violations of any federal or state health care program requirements by the physician, (iii) any threatened, proposed, or actual revocation of license/certification, suspension, exclusion, or debarment of the physician, (iv) any subpoena, legal process, civil investigative demand, discovery request, or similar request for information involving the physician, and/or (v) any facts, allegations or situations of a similar nature. 

To be listed on the UroLift.com Physician Locator, a physician must: 

• Complete the UroLift™ Proficiency Pathway (UPP)¹ or equivalent training², and  
• Complete at least 15 cases in the last 12-month period. 
  

A physician receiving UroLift™ training in Residency must:

• Complete the UroLift™ Residency Proficiency Pathway (RPP)³
   certification
• Complete at least 15 cases in the last 12-month period
  • Cases can be in residency or a mix of residency and in practice
  • Complete a Teleflex Clinical Affairs refresher training and proctored
     case in practice

To remain on the Physician Locator, a physician must complete at least 8 cases in the 12-month review period

The 12-month review period is based on the date the physician is added to the Physician Locator. A physician will be removed from the Physician Locator if they have not met the above requirements. 

To be added back on the Physician Locator, a physician must complete at least 8 cases in the last 12 months. 

Please note: Physicians who have achieved Center of Excellence designation do not need to requalify separately for the Physician Locator each year and are listed on the Physician Locator as part of their COE qualification. Should a physician not renew their designation for the Center of Excellence program, they must meet the Physician Locator annual requirements to remain listed on UroLift.com.

2 Physicians who were trained prior to March 2017 received equivalent surgeon training to the requirements outlined above, including instructional didactic sessions, clinical video review and hands-on UroLift™ System use. The current and prior training programs focus on identifying appropriate patient candidates; preparation and operation of the UroLift™ System; and surgical technique. 

3. RPP is a residency surgeon-training program that includes modular, didactic and hands-on device training and guided completion of 15 cases. For more information about RPP, contact your Teleflex representative.

The UroLift™ System Center of Excellence (COE) Program is designed to enhance patient outcomes and experience. Urologists included in the UroLift™ System COE Program have achieved a higher level of training with the UroLift™ System and demonstrated a commitment to care for men suffering from symptoms associated with Benign Prostatic Hyperplasia (BPH).  

To qualify for our UroLift™ COE designation, a physician must: 

1. Complete the UroLift™ Proficiency Pathway (UPP)¹ or equivalent training², at time of initial designation, 
2. Participate in the 12-month period prior to submission for designation or re-designation⁴ in a minimum of: 
   1. Two Teleflex Clinical and Medical Affairs-led educational events for initial designation, and 
   2. One Teleflex Clinical and Medical Affairs-led education event per year for re-designation
3. Complete a minimum of 60 UroLift™ System cases in the 12-month period prior to submission for designation or re-designation^5,6 and
4. Submit the following data on a minimum of:
   1. 30 UroLift™ System cases completed in the 12-month period prior to submission for designation or
   2. 15 UroLift™ System cases for re-designation⁵,
   3. de-identified data evidencing: 
      1. Pre- and post-procedure International Prostate Symptom Scores (IPSS) demonstrating a mean improvement in IPSS of 40% or greater compared to baseline average; 
      2. The dates of the UroLift™ treatment, pre- and post-procedure IPSS assessments; and 
      3. Use of BPH medication at the time of the pre- and post-procedure assessments. 

If a physician does not renew their designation as a UroLift™ System COE physician, the physician will be notified, the physician’s status as a COE will be suspended, and their COE insignia will be removed from the Physician Locator. That physician will be reviewed by Teleflex to determine whether they meet the requirements to remain on the Physician Locator. If the physician does not meet the Physician Locator requirements, the physician will be notified and removed from the Physician Locator entirely⁷.

4 In situations where elective procedures were impacted due to a national health crisis, the 12-month period for designation or re-designation may be addressed to provide accommodation. Therefore, the assessment period may not be consecutive months.

6 Completion may include proctored cases in connection with a Teleflex Clinical and Medical Affairs-led peer-to-peer educational event. 

7 Physicians that were removed from the Physician Locator prior to September 2018 need to meet the initial add requirements to be added back.
